Services

Supported Living Services – We work for YOU

Supported Living Services are available to persons whose needs can be met within a home setting, and are always customized to meet the specific needs of each person.

Specific services are comprehensive, flexible, personalized, and directed towards ensuring we support people in meeting their needs and goals, pursuing their interests and reaching their full potential. Services are always able to be modified as needed to meet changing needs of person’s life and interests.

The person served is always the guiding force in his/her planning. A person’s strengths, preferences, interest, desires, goals are discussed and examined, and local options within that person’s community of choice are evaluated; All options are presented, and decisions made by the person are always honored.

Our goal is for our services to promote opportunities for learning, skill-building, creativity, uniqueness, health, happiness, and encouragement towards the pursuit of one’s interests and the realization of one’s potential. Guided by the person receiving services, his/her family, guardian, friends, and any support agencies, we coordinate these needs and desires with local resources, so as to preserve and enhance connections to natural supports, but also help locate, create, maintain and expand on opportunities for community involvement and participation in innovative, creative, safe, healthy, and productive ways.

Persons served retain control of his or her home environment, including who visits and works there. We work to ensure a high quality of life and a high quality of service,
offering dignity, empowerment, respect, the ability to make informed decisions, ensure and protect a person’s rights, and to provide humane care at all times.

Residential Services

Residential Services are available to persons whose needs can best be met within a communal residential setting. Programs are customized to meet the specific needs of each person.

Specific services are comprehensive, flexible, personalized, and directed towards ensuring we support people in meeting their needs and goals, pursuing their interests and reaching their full potential. Services are always modified as needed to meet changing needs of person’s life and interests.

While Bayberry does support any transitional planning for our residents to a more independent setting, transition is not necessarily the goal of each person. Each person’s care is tailored to his or her own unique needs and abilities. Instruction in and support for all aspects of all living skills are available at each home, to the degree required by each person.

Bayberry is committed to full support of client choice and family involvement in the planning of services for persons in residential service. Bayberry reviews these service plans, both regularly and on an as-needed basis, as well as participating in any IPP meetings that outside agencies may require, so as to stay as informed of and supportive to all aspects of the client’s care, support, and education.

Our Residential Services are provided in two homes in quiet residential neighborhoods in the town of Napa, California. Each home serves six persons, and is licensed as an Intermediate Care Facility/Developmental Disability-Habilitation (ICF/DD-H) by the California Department of Health Services.

Bayberry House

The Bayberry House, opened in partnership with the Department of Housing & Urban Development, is a large seven bedroom home in Napa. Each resident has his or her own bedroom. In addition to customizing each person’s individual service plan, this program maintains a very active schedule of community integrated activities. This home is licensed by the Department of Health Services to serve up to six ambulatory adults, who have moderate to severe behavior problems.

Saratoga House

The Saratoga House had a major, twenty five thousand dollar renovation and improvement performed with the help of the Napa Housing Authority. Each person who lives there shares a room with one other resident. In addition to customizing each person’s individual service plan, this program specializes in comprehensive behavioral management services and an integrated health care management approach. This home is licensed by the Department of Health Services to serve up to six ambulatory adults, who have moderate to severe behavior problems.
